Initial Consultation
A consultation appointment will consist of an explanation of your diagnosis and all possible treatment options. If scheduled in conjunction with treatment, your procedure may begin shortly after the consultation. Many times, treatment can be completed in the same visit. However, a complex medical history or treatment plan may require an evaluation with a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Your treatment referral slip and any X-rays, if applicable.
- A list of prescription, herbal and over-the-counter medications you are presently taking.
- If you have dental insurance, please bring your plan’s identification card(s) and any necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or legal guardian for all visits.
X-Rays
If your dentist has taken x-rays, you may request that they forward them to our office by mail or email. If there is not enough time, please pick them up and bring them to our office. If additional images are necessary, they can be taken at our office. Dr. Engel or Dr. Martin may require additional digital x-rays should further information be needed. We use x-rays as our ‘blue print’ for diagnosis and treatment. Our digital x-rays use up to 90% less radiation versus conventional films.
